Kidney function tests explained: creatinine, eGFR and urine protein

Kidney disease is often silent. Two simple tests, a blood creatinine and a urine protein check, show how well the kidneys are working long before symptoms appear.

Serum creatinine

A waste product the kidneys clear from the blood. A higher level usually means the kidneys are filtering less well. One raised result is often repeated to confirm it.

eGFR

An estimate of how well the kidneys filter, worked out from creatinine, age and sex. Above 90 is normal; persistently below 60 for three months suggests chronic kidney disease.

Urine protein or ACR

Healthy kidneys keep protein out of the urine. Protein in the urine is an early sign of kidney damage, especially in diabetes and high blood pressure.

Who should be tested

People with diabetes, high blood pressure, heart disease, a family history of kidney disease, and those taking long-term painkillers or certain other medicines.

When to see a nephrologist

A falling eGFR, protein in the urine, swelling of the legs or face, or high blood pressure that is hard to control.
