Piles, fissure or fistula? How to tell them apart and how each is treated

Piles, fissure and fistula all cause trouble around the back passage, but they are different problems with different treatments. A short examination by a proctologist tells them apart.

Piles (haemorrhoids)

Piles are swollen blood vessels inside or at the edge of the back passage. The usual signs are bright red blood on the stool or tissue, usually without pain, and a lump that may come out during motion and go back in.

Fissure

A fissure is a small tear in the lining of the back passage. It causes sharp, cutting pain during and after passing stool, often lasting an hour or more, sometimes with a streak of blood. Hard stools and straining are the usual triggers.

Fistula

A fistula is a small tunnel between the inside of the back passage and the skin nearby, usually after an abscess. It shows as a painful swelling or a small opening that keeps leaking pus or blood and does not heal on its own.

How each is treated

Early piles and new fissures often settle with more fibre and water, stool softeners and ointments. Piles that keep bleeding or coming out, fissures that do not heal and almost all fistulas usually need a procedure. Laser is one option for many patients; the doctor will say whether it suits you.

When to see a doctor

See a doctor for any bleeding from the back passage, even if you think it is piles, because other bowel conditions can cause bleeding too. Go the same day for heavy bleeding, fever with a painful swelling, or pain that stops you sitting.

Questions patients ask

Where can I see a piles doctor in Chitradurga?

Disha Hospital in VP Extension, Chitradurga runs a laser proctology clinic for piles, fissure and fistula. OPD is Monday to Saturday, 9 am to 5 pm. Call +91 89517 23114 to book.

Is laser treatment for piles painful?

Laser treatment is done under anaesthesia, so you do not feel the procedure. Most patients have less pain afterwards than with conventional surgery, but some soreness for a few days is normal.

Can piles go away without surgery?

Yes, early piles often settle with diet changes, more water, stool softeners and avoiding straining. A procedure is advised when piles keep bleeding, keep coming out, or do not respond to these measures.

Is fistula treated with laser?

Laser is one of the options for some fistulas. The right treatment depends on where the tunnel runs, which the surgeon checks during examination and, if needed, a scan.
