Thyroid surgery: who needs it and what to expect

Most thyroid swellings do not need surgery. An operation is advised when a nodule looks suspicious, a goitre presses on the windpipe or food pipe, or an overactive thyroid cannot be controlled.

Reasons surgery may be advised

A nodule that tests suggest could be cancer, a large goitre causing pressure, trouble breathing or swallowing, a swelling that keeps growing, or an overactive thyroid not controlled by medicines.

Tests before deciding

Thyroid blood tests, an ultrasound of the neck and often a fine-needle test (FNAC) that takes a few cells from the lump to check under a microscope.

The operation

Either half (hemithyroidectomy) or all (total thyroidectomy) of the gland is removed through a cut in a natural crease of the neck, under general anaesthesia.

After surgery

Most patients stay in hospital for one to two days. A hoarse voice or tingling from low calcium can happen for a short time; the team checks for both.

Long-term

After total thyroidectomy you take a thyroid tablet every day for life. After half, many people do not need one. Regular blood tests guide the dose.
